# Medicare Advantage (Part C)

Medicare Advantage replaces Original Medicare with a private plan (often HMO or PPO) that must cover at least what Part A and B cover, usually with networks, copays, and often Part D drugs built in.

## FAQ

- Is Medicare Advantage really free? You usually still pay the Part B premium plus copays when you use care. "$0 premium" is the plan premium only.
- Can I switch back to Original Medicare? Yes, during Annual Enrollment (Oct 15–Dec 7) or MA Open Enrollment (Jan 1–Mar 31) if already in an MA plan.
- Does it cover drugs? Most MA-PD plans include Part D; confirm before enrolling.
- Can I have MA and Medigap together? No.
- Do all doctors accept it? Only the plan network (except true emergencies).

# Medicare Supplement (Medigap)

Medigap policies help pay Original Medicare cost-sharing. Standardized lettered plans. Sold today, they do not include Part D drug coverage.

Best time to buy: 6-month Medigap Open Enrollment starting the month you are 65+ and enrolled in Part B (guaranteed issue). Oregon and Washington have extra switching protections that national guides often omit; see the state rules guide.

You cannot have Medigap and Medicare Advantage at the same time.

# Medicare Part D

Standalone prescription drug coverage, or included in many Medicare Advantage (MA-PD) plans.

The coverage gap ("donut hole") ended January 1, 2025. There is a deductible, initial coverage, then an annual out-of-pocket cap. The Medicare Prescription Payment Plan spreads pharmacy costs across monthly payments; it does not reduce the total owed.

Going without creditable drug coverage at first eligibility can trigger a permanent late-enrollment penalty. Plan changes are generally limited to IEP, Annual Enrollment (Oct 15–Dec 7), or a Special Enrollment Period.

# ACA Open Enrollment (Oregon & Washington)

For 2027 coverage:

- Washington: Washington Healthplanfinder. Open Enrollment November 1, 2026 through January 15, 2027. Sign up by December 31 for January 1 coverage; January 1–15 for February 1 coverage.
- Oregon: Explore Health (replaces HealthCare.gov starting November 1, 2026). Enroll November 1–December 15 for January 1 coverage; December 16–January 15 for February 1 coverage.

Missing Open Enrollment usually means waiting unless a qualifying life event opens a Special Enrollment Period. Auto-renewal can leave you on a worse plan; re-shop yearly.

# ACA metal tiers

Metal tiers describe average cost-sharing, not care quality.

- Bronze: lower premium, higher deductible
- Silver: only tier eligible for Cost-Sharing Reductions (CSR) if income qualifies — often the best real-world value
- Gold: higher premium, lower cost at the point of care
- Platinum: rarely offered in Oregon and Washington
- Catastrophic: generally under 30 or hardship exemption; not subsidy-eligible

# Special Enrollment Periods (ACA)

A qualifying life event (job loss, marriage, move, birth, loss of coverage, etc.) generally opens a 60-day window from the event date. Miss it and you typically wait for Open Enrollment.

CMS ended the monthly SEP for households at or below 150% FPL on August 25, 2025 for plan year 2026. CMS has said that SEP is scheduled to return for plan year 2027 (coverage beginning January 1, 2027). Confirm before assuming year-round enrollment is available.

Medicare has a separate SEP system with different triggers.

# Oregon and Washington insurance rules

State law adds consumer protections beyond the federal baseline.

Medigap: Washington generally allows year-round guaranteed-issue switching among comparable standardized plans and uses community rating. Oregon has a birthday-rule style switching right for many Medigap plans (confirm current OAR).

Short-term limited-duration plans: Washington caps them at 3 months, non-renewable, and no more than 3 months in any 12-month period. Availability still depends on carriers offering a compliant plan.

Medicaid names: Oregon Health Plan (OHP) in Oregon.

Residence (vote, license, taxes) usually decides which state's rules apply if someone splits time between OR and WA.
